I am an M.D/Ph.D. student and work as a research assistant for the director of a division of the school of medicine who is an M.D. He assigned me to research a certain topic and gave me no guidelines or guidance as to how to do it. Nevertheless, I did the research and wrote it up. My supervisor liked the report and said that he thought it was so good that “I would like to offer you the opportunity to publish (...) it and list you as the primary author.” Some bells went off when he so grandly offered to let me author the report for which I had done 100% of research and writing. I consulted some other people in the field and they said that, as long as I was the primary author, it was legitimate for him to list himself as secondary author if he did some editing later. After editing the abstract only, he e-mailed his revisions to me and in a note at the bottom he asked me what I thought of his revised author order. His name was first, mine second, and the name of his girlfriend was third. I was shocked by what seemed to be a case of unethical author attribution and confronted him asking why he changed the order when we had agreed that I was primary author. However, the traditional construction of patient medication adherence is oversimplified, myopic, and ethically problematic. Adherence relies on existing social power structures and western normative assumptions about the proper roles of patients and providers, and principally focuses on patient variables, obscuring the powerful socioeconomic and institutional influences on (...) behaviour. Some professionals advocate for alternate approaches to adherence, but many of the available alternatives remain conceptually underdeveloped. Using HIV/aids as an exemplar, this paper presents medication practice as a theoretical reconstruction and explicates its conceptual and ethical evolution. We first propose that one of these alternatives, medication practice, broadens the understanding of individuals’ medication‐taking behaviour, speaks to the inherent power inequities in the patient‐provider interaction, and addresses the ethical shortcomings in the traditional construal. We then integrate medication practice with feminist thought, further validating individuals’ situated knowledge, choices, and multiple roles; more fully recognizing the individual as a multidimensional, autonomous human being; and reducing notions of obedience and deference to authority. Blame is thus extricated from the healthcare relationship, reshaping the traditionally adversarial components of the interaction, and eliminating the view of adherence as a patient problem in need of patient‐centred interventions. The relevance of the research topic. After the World War II the leaders and intellectuals of the world were forced to look for new principles and methods of management. New theories for analysis of complex systems appeared. The D. Meadows’ group from the Club of Rome and other analysts discovered the fact of the deepening humanity in an irreversible and deadly crisis. Up to now, economists and politicians have not offered a way to guarantee rescue. However, there is a consensus (...) that hopes can be linked to the development of education and science in the world. The phrase “internationalization of higher education ” is used more and more often, and the number of scientific works is growing rapidly. However, in our opinion, the phenomenon of IHE is perceived superficially because of inattention to the latest scientific achievements, which are not followed by humanities scholars. This caused our interest and stimulated the desire to prove that global process of IHE started in accordance with the laws of the evolution of industrial production. The purpose of our study is to apply the theory of analysis of very large systems to the phenomena of kinds and variants internationalization to obtain the most probable scenarios for human progress. The task of the study is seen in the critical analysis of the trend we noted in the process of training the PhD-level professional in space and time and transforming the figure of a young worker from a national to an international “product” of the systems of education and training. The methodology of the research was dictated by the chosen goal and tasks under the pressure of the grandiose object – a complex of educational systems in a globalized planet environment. This is due to the use of the means of many sciences, theories of systemic research and a multidisciplinary approach in the conditions of global evolutionism. The results of the study in their essence, are still based on historical methods, in conjunction with the achievements of young sciences – synergetics, theories of complex systems, and others. An original table was created to match the agrarian, industrial and informational societies – those within which the system of higher education had been developed. The direction of its characterization has proven that the autonomy of universities has been diminished at present, the size of the whole system and its orientation towards political and economic demands have increased. It is indicated at the time when the high school began to lose its national character and to acquire features of internationality. It is noted that this coincided with the growth of the volume and impact of globalization and its study mainly within the humanities. The widespread representation of the interaction in higher education with the influences of globalization and internationalization, the basic definitions and differentiation, etc. is presented. The newest data on internationalization within the universities and far beyond them in state and interstate institutions are presented. The conclusions and recommendations formulated and proved the main hypothesis of our study and its main result. It consists in the fact that the higher school in general began to reproduce the path of industry with its variant of internationalization – a combination of products of many countries. We argue that in higher education the division of time and space of educational programs, institutions, places of internship, etc. began. Instead of ensuring PhD competences within a single state, basic schooling is found to be more profitable in one place, the profile secondary is in the other, the baccalaureate or master’s program in the country “A”, postgraduate study somewhere in “B”, the final professional development in the best world’s centers of MBA or some other training in London or Harvard. From this follows the following conclusion: Ukraine’s resources are already inadequate for the preparation of national personnel of the highest world quality. According to grounded cognition, words whose semantics contain sensory-motor features activate sensory-motor simulations, which, in turn, interact with spatial responses to produce grounded congruency effects. Growing evidence shows these congruency effects do not always occur, suggesting instead that the grounded features in a word's meaning do not become active automatically across contexts. Researchers sometimes use this as evidence that concepts are not grounded, further concluding that grounded information is peripheral to the amodal cores of concepts. We first review broad evidence (...) that words do not have conceptual cores, and that even the most salient features in a word's meaning are not activated automatically. Then, in three experiments, we provide further evidence that grounded congruency effects rely dynamically on context, with the central grounded features in a concept becoming active only when the current context makes them salient. Even when grounded features are central to a word's meaning, their activation depends on task conditions. (shrink)

Rationale, aims and objectives To compare the pharmacotherapeutic approaches to diabetic hypertension of family physicians (FPs) and general practitioners (GPs). Methods A retrospective prescription-based study was conducted in 15 out of a total of 20 health centres, involving 115 primary care physicians – 77 FPs and 38 GPs, representing 74% of the primary care physicians of Bahrain. Prescriptions were collected during May and June 2000 to comprise a study population of 1266 diabetic-hypertensive patients. Results As monotherapy, angiotensin-converting enzyme (ACE) inhibitors (...) (37.9%) and β-blockers (38.3%) were the most commonly prescribed classes of antihypertensives by FPs and GPs, respectively. Calcium channel blockers (CCBs) were ranked third by both categories of physicians. For two-drug combinations, a β-blocker and an ACE inhibitor was the combination of choice for both physician categories. Patients managed by the FPs were more likely to receive a β-blocker–CCB combination (17.4 vs. 14.9%) or a diuretic–ACE inhibitor combination (16.7 vs. 11.4%) and less likely to receive a β-blocker–diuretic combination (11.8 vs. 16.7%) than those managed by the GPs. The proportion of patients receiving antihypertensive combinations was 40.6 and 38.5% for FPs and GPs, respectively. While the GPs prescribed CCB as a monotherapy to the elderly most often, the FPs choice was a β-blocker. Diuretics were less preferred by both FPs and GPs. β-Blocker–ACE inhibitor was again the most preferred combination of both FPs and GPs. FPs prescribed CCB–β-blocker combinations more often than GPs (P = 0.01), whereas CCB–ACE inhibitor combinations were less preferred (P = 0.09). A trend towards excessive use of short-acting nifedipine as monotherapy for elderly patients, both by FPs and by GPs, was noticed. Glibenclamide, alone or in combination with metformin, was the foremost antidiabetic drug prescribed by FPs and GPs. Middle-aged (45–64 years) patients seen by GPs were more likely to receive glibenclamide than those treated by FPs (P = 0.001) and less likely to receive gliclazide (P = 0.01). Combinations of a β-blocker with either glibenclamide or insulin were prescribed considerably more often by GPs.Conclusions Within the same practice setting, a substantial difference was observed between FPs and GPs in terms of preference for different classes of drugs in the management of diabetic hypertension. The compliance of both FPs and GPs was suboptimal; overall, the compliance of the FPs was closer to the recommended guidelines, however. Educational programmes should specifically address these inadequacies in order to improve the quality of health care. (shrink)

Health care organizations are increasingly asked to show accountability about their performance. This paper proposes that accountability can best be achieved through evaluative methods that are based on evidence regarding the relationship between processes of care and expected outcomes. Root cause analysis (RCA) is used as an illustration of how a generic method of inquiring can be transformed into an ongoing monitoring, evaluation, user education and accountability strategy. In response to Casasanto, Brookshire, and Ivry, we address four points: First, we engaged in conceptual replications of Brookshire, Casasanto, and Ivry, not direct replications. Second, we did not question the validity of Brookshire et al.'s results, nor the similar findings of other researchers, but instead explained divergent findings within an integrated theoretical framework. Third, challenges to the construct of automaticity, including ours, were widespread, long before Brookshire et al.'s article. Fourth, the planned comparisons that we reported tested our theoretical (...) claims and offered strong evidence for them. (shrink)

In several recent publications, Professor Adolf Grünbaum has inveighed against the conventionalism of writers like Einstein, Poincaré, Quine and especially Duhem. Specifically, Grünbaum has assailed the view that a single hypothesis can never be conclusively falsified. Grünbaum claims that the conventionalists’ insistence on the immunity of hypotheses from falsification is neither logically valid nor scientifically sound. Directing the weight of his argument against Duhem, Grünbaum launches a two-pronged attack. He insists, first, that conclusive falsifying experiments are possible, suggesting that Duhem's (...) denial of such experiments is a logical non-sequitur. He then proceeds to show that, more than being merely possible, crucial falsifying experiments have occurred in physics. I do not intend to make a logical point against Grünbaum's critique so much as an historical and exegetical one. Put briefly, I believe that he has misconstrued Duhem's views on falsifiability and that the logical blunder which he discusses should not be ascribed to Duhem, but rather to those who have made Duhem's conventionalism into the doctrine which Grünbaum attacks. Whether there are any writers who accept the view he imputes to Duhem, or whether he is exploiting “straw-men” to give weight to an otherwise trivial argument is an open question. We present a one per cent measurement of the cosmic distance scale from the detections of the baryon acoustic oscillations in the clustering of galaxies from the Baryon Oscillation Spectroscopic Survey, which is part of the Sloan Digital Sky Survey III. Our results come from the Data Release 11 sample, containing nearly one million galaxies and covering approximately 8500 square degrees and the redshift range 0.2 < z < 0.7. We also compare these results with those from the publicly released (...) DR9 and DR10 samples. Assuming a concordance Λ cold dark matter cosmological model, the DR11 sample covers a volume of 13 Gpc3 and is the largest region of the Universe ever surveyed at this density. We measure the correlation function and power spectrum, including density-field reconstruction of the BAO feature. The acoustic features are detected at a significance of over 7σ in both the correlation function and power spectrum. Fitting for the position of the acoustic features measures the distance relative to the sound horizon at the drag epoch, rd, which has a value of rd,fid = 149.28 Mpc in our fiducial cosmology. We find DV = at z = 0.32 and DV = at z = 0.57. At 1.0 per cent, this latter measure is the most precise distance constraint ever obtained from a galaxy survey. Separating the clustering along and transverse to the line of sight yields measurements at z = 0.57 of DA = and H =. Our measurements of the distance scale are in good agreement with previous BAO measurements and with the predictions from cosmic microwave background data for a spatially flat CDM model with a cosmological constant. © 2014 The Author Published by Oxford University Press on behalf of the Royal Astronomical Society. While there is now considerable anxiety about whether the psychological theory presupposed by virtue ethics is empirically sustainable, analogous issues have received little attention in the virtue epistemology literature. This paper argues that virtue epistemology encounters challenges reminiscent of those recently encountered by virtue ethics: just as seemingly trivial variation in context provokes unsettling variation in patterns of moral behavior, trivial variation in context elicits unsettling variation in patterns of cognitive functioning. Insofar as reliability is a condition on epistemic virtue, (...) we have reason to doubt that human beings possess the cognitive materials required for epistemic virtue, and thereby reason to think that virtue epistemology is threatened by skepticism. We conclude that while virtue epistemology has resources for addressing this challenge, exploiting these resources forces tradeoffs between empirical and normative adequacy. (shrink)

This paper explores the use of video news releases (VNRs) without source disclosure from legal and ethical perspectives. In light of current regulatory debates regarding VNRs, the paper first examines whether journalists' use of corporate VNRs without source disclosure violates FCC regulations. It then questions the ethics of using such VNRs by examining the current code of ethics for both the public relations practitioners creating VNRs and the news organizations airing them. The paper uses the ethical construct of transparency to (...) further examine the ethics of VNR production and exhibition without source disclosure. It finds that regardless of whether the use of VNRs without source disclosure violates current sponsorship rules, the use of such VNRs not only violates codes of ethics but also transparency, which includes values such as accountability, credibility, trust, respect, truth and honesty, reason, freedom, dignity, and duty. Multiteam systems are complex organizational forms comprising interdependent teams that work towards their own proximal goals within and across teams to also accomplish a shared superordinate goal. MTSs operate within high-stakes, dangerous contexts with high consequences for suboptimal performance. We answer calls for nuanced exploration and cross-context comparison of MTSs “in the wild” by leveraging the MTS action sub-phase behavioral taxonomy to determine where and how MTS failures occur. To our knowledge, this is the first study to also examine how (...) key MTS attributes influence MTS processes and performance. We conducted historiometric analysis on 40 cases of failed MTS performance across various contexts to uncover patterns of within- and between-team behaviors of failing MTSs, resulting in four themes. First, component teams of failing MTSs over-engaged in within-team alignment behaviors by enacting acting, monitoring, and recalibrating behaviors more often within than between teams. Second, failing MTSs over-focused on acting behaviors and tended to not fully enact the action sub-phase cycle. Third and fourth, boundary status and goal type exacerbated these behavioral patterns, as external and physical MTSs were less likely to enact sufficient between-team behaviors or fully enact the action sub-phase cycle compared to internal and intellectual MTSs. We propose entrainment as a mechanism for facilitating MTS performance wherein specific, cyclical behavioral patterns enacted by teams align to facilitate goal achievement via three multilevel behavioral cycles. We argue that the degree to which these cycles are aligned both between teams and with the overarching MTS goal determines whether and how an MTS fails. Our findings add nuance beyond single-context MTS studies by showing that the identified behavioral patterns hold both across contexts and almost all types of MTS action-phase behaviors. We show that these patterns vary by MTS boundary status and goal type. Our findings inform MTS training best practices, which should be structured to integrate all component teams and tailored to both MTS attributes and situation type. (shrink)

Surveys are used extensively in social research and, despite a lack of conclusive evidence of their ‘representativeness,’ probability internet panel surveys are being increasingly used to make inferences about knowledge, attitude and behavior in the general population regarding a range of socially relevant issues. A large-scale survey of Australian public attitudes and behavior toward the red meat industry was undertaken. Samples were obtained using a random digit dialing telephone survey and a PIP survey to examine differences between the two samples (...) regarding attitudes and behavior relating to livestock use and welfare. There was little difference in demographics between the CATI and the PANEL surveys apart from highest level of education. However, there were differences between the two samples in both attitudes and behavior toward the red meat industry after controlling for education levels. The PANEL respondents gave generally more conservative responses than did the CATI respondents in the sense that they were more positive toward the livestock industries and animal welfare within these industries. Differences were also found between the respondents of the two samples regarding behavior that relates to the red meat industry, both community and consumer behavior. PANEL respondents were less engaged in community behaviors performed in opposition of the red meat industry when compared with the CATI sample. The majority of CATI and PANEL respondents were red meat eaters and there was no difference between respondents of the two samples in relation to red meat consumption, however, there were fewer vegetarians and vegans in the PANEL survey. Possible reasons for the observed differences are discussed, however, a definitive answer will depend on further research to identify the specific psychological factors that differ between samples derived from different survey methodologies. A number of anthropologists have argued that religious concepts are minimally counterintuitive and that this gives them mnemic advantages. This paper addresses the question of why people have the memory architecture that results in such concepts being more memorable than other types of concepts by pointing out the benefits of a memory structure that leads to better recall for minimally counterintuitive concepts and by showing how such benefits emerge in the real‐time processing of comprehending narratives such as folk tales. This (...) model suggests that memorability is not an inherent property of a concept; rather it is a property of the concept, the context in which the concept is presented, and the background knowledge that the comprehendor possesses about the concept. The model predicts how memorability of a concept should change if the context containing the concept were changed. The paper also presents the results of experiments carried out to test these predictions. (shrink)

Machery proposes that the construct of detracts from research progress. However, ignoring development also detracts from research progress. Developmental research has advanced our understanding of how concepts are acquired and thus is essential to a complete theory. We propose a framework that both accounts for development and holds great promise as a new direction for thinking about concepts.

After a chapter which is an introduction to and summary of the rest of the book, chapter 2 begins by criticizing various attempts to do away with theories, such as the Reichenbach-Salmon conception of theoretical truth in terms of observational consequences, and the Ramsey strategy of replacing first-order theoretical sentences by second-order nontheoretical ones; it then argues against hypothetico-deductivist theories of confirmation on the grounds that they are unable to handle the relevance of evidence to theory, whether or not other (...) formal constraints or even nonformal ones such as simplicity and explanatory power are added to those theories; it ends with an exposition of the Reichenbach-Carnap conventionalist thesis that the confirmation of hypotheses with evidence presupposes analytic principles connecting hypothetical and empirical concepts. Chapter 3 criticizes Bayesian theories of confirmation on such grounds as that the a priori arguments for Bayesian restrictions on beliefs are insufficient; that the Bayesian explanations of notions like variety of evidence, ad hocness, and simplicity are inadequate; and that they cannot account, without drastic revisions, for the relevance of evidence to theory, especially old evidence to new theory. Chapter 4 is an unsympathetic account of historically oriented philosophies of science; Lakatos’s notion of progressive research programs and Laudan’s theory of problem-solving effectiveness are found to be insufficiently precise and dubbed examples of "new fuzziness", while the Sneed-Stegmüller dynamics of theories is regarded as "a paradigm of misplaced rigor" ; and all three are criticized as unable to explicate the concept of evidential relevance. Chapter 5 is a formal and informal elaboration of the basic idea in Glymour’s account of theory and evidence, namely that confirmation or support is a three-term relation among evidence, hypothesis, and theory, so that evidence confirms an hypothesis only relative to a theory, and such that evidence e confirms hypothesis h relative to T if T with e implies, but T without e does not imply, an instance of h, and T with e does not imply a comparable hypothesis h’ different from h; the most interesting part of this chapter is a series of discussions showing how such an idea makes sense of the "methodological truisms" that it is possible selectively to test certain parts of a theory but not others, that a theory is supported better by a greater variety of evidence, that redundant or physically meaningless quantities are an undesirable feature of theories, that there are no inviolable rules, that a "white shoe" does not confirm "all ravens are black," that a theory is more useful and better supported than the set of its observational consequences, and that the hypothetico-deductive method often works. Since Glymour claims no originality for the conception of this general account, but only for its description, he plausibly thinks it necessary to apply it to some important historical cases of actual science; he does so in the next chapter by discussing at some length the structure and merits of Ptolemaic and Copernican astronomy, of Newton’s universal gravitation, of nineteenth century atomism, of Freud’s Rat Man, and of general relativity. For example, he argues convincingly that Copernicanism was better supported by the evidence available even before the telescope because it alone, and not the Ptolemaic theory, can determine some planetary properties which are presupposed by both theories, can test and confirm some common hypotheses, and can be confirmed in an essential rather than incidental manner; and though Glymour admits that the Copernicans tended to argue in terms of explanatory rather than confirmatory superiority, he sketches an interesting account of the relation between explanation and confirmation which strengthens his case. He also analyzes the text at the beginning of book 3 of Newton’s Principia and compiles a reconstruction of the intricate details of the inferential structure of the argument to universal gravitation; his impressive reconstruction clearly shows that Newton’s method is neither the inductivist one he himself thought he was following, nor the one attributed to him by Pierre Duhem, but rather one in accordance with Glymour’s so-called "bootstrap strategy" of repeatedly using the evidence from Kepler’s second and third laws, from lunar data, and from falling bodies, together with the theory consisting of the second and third laws of motion, to arrive at increasingly general instances of the principle of universal gravitation. Glymour’s account of arguments for and against the atomic theory from Thomson to Cannizzaro is his longest historical example, but he himself admits that this illustration of the bootstrap strategy is "certainly not the neatest", and rather "diffuse, and perhaps disconnected". Chapter 7 is a detailed application of the same principles to the technique known among social scientists as "causal modeling"; since such "causal models" are essentially systems of algebraic equations, Glymour’s account shows indirectly that the bootstrap strategy is applicable to any scientific theory presented as a set of algebraic equations; this, of course is not surprising, since the formal considerations with which Glymour begins the exposition of his theory in chapter 5 involve systems of algebraic equations. Chapter 8 is an attempt to apply Glymour’s theory of evidence to the very common scientific practice of curve-fitting, where one measures values of two or more quantities and then infers a functional relationship; Glymour argues that simpler polynomials are preferred because they are tested more frequently and more severely by the data and because they are more informative; but then he criticizes his own explanation and concludes that "the bootstrap procedures of chapter 5 do not seem to help the problem much more than a whit" ; this is admirably honest, but if one wants an additional logical or methodological rationale, then one would perhaps have to go to the meta-level and say that Glymour is here testing his own theory, and he is more interested in a further illustration of the bootstrap methodology even with a failed test, than he is to claim universal or exclusive validity for it. In chapter 9 Glymour uses his bootstrap theory of confirmation to criticize geometrical conventionalism; he considers the alternative theories of space or space-time proposed by conventionalists to show the radical undetermination of geometries by evidence, for example, a non-Euclidean geometry with no universal force instead of Newtonian gravitation with Euclidean geometry; he argues that such alternatives are simply not as well tested as the originals and that their equivalence to the originals is questionable. The last chapter is a brief discussion of further problems worthy of future investigation. (shrink)

Opinion leaders within the community may lead debate on animal welfare issues and provide a path for information to their social networks. However, little is known about OLs’ attitudes, activities conducted to express their views about animal welfare and whether they are well informed, or not, about husbandry practices in the red meat industry. This study aimed to identify OLs in the general public and among producers and compare OLs and non-OLs’ attitudes, knowledge and actions to express their views about (...) the red meat industry. Two questionnaires, one for the Australian general public and one for Australian red meat producers, were developed to identify general attitudes. From these questionnaires, OLs were identified using a two-step cluster analysis. Subsequently, a sub-sample of 19 OLs participated in a follow-up phone interview. Results disclosed some clear OLs’ characteristics. Public OLs held more negative perceptions of the red meat industry and perceived they had more knowledge about husbandry procedures. However, their actual knowledge about animal husbandry was not different from non-OLs. Public OLs also used and trusted social and internet media more than did non-OLs. In the producer group, a large percentage of OLs were identified. Producer OLs had more actual knowledge about animal husbandry and engaged in more behaviours to express dissatisfaction with the industry than non-OLs. Unlike the public respondents, this group used conventional media more than social and internet media, and their levels of trust in all kinds of media were low. While there were clear differences, both groups believed that is important to increase communication and educate about farm practices. This may present an opportunity to develop an opinion leader intervention strategy where informed OLs could later disseminate accurate information to their social networks. Further studies should test if sustained and facilitated educational sessions between public and producer OLs can assist in increasing communication, knowledge and perhaps, may assist in achieving convergence of concerns and expectations between both groups. Curbside ethics consultations occur when an ethics consultant provides guidance to a party who seeks assistance over ethical concerns in a case, without the consultant involving other stakeholders, conducting his or her own comprehensive review of the case, or writing a chart note. Some have argued that curbside consultation is problematic because the consultant, in focusing on a single narrative offered by the party seeking advice, necessarily fails to account for the full range of moral perspectives. Their concern is that (...) any guidance offered by the ethics consultant will privilege and empower one party’s viewpoint over—and to the exclusion of—other stakeholders. This could lead to serious harms, such as the ethicist being reduced to a means to an end for a clinician seeking to achieve his or her own preferred outcome, the ethicist denying the broader array of stakeholders input in the process, or the ethicist providing wrongheaded or biased advice, posing dangers to the ethical quality of decision-making. Although these concerns are important and must be addressed, we suggest that they are manageable. This paper proposes using conflict coaching, a practice developed within the discipline of conflict management, to mitigate the risks posed by curbside consultation, and thereby create new “spaces” for moral discourse in the care of patients. Thinking of curbside consultations as an opportunity for “clinical ethics conflict coaching” can more fully integrate ethics committee members into the daily ethics of patient care and reduce the frequency of ethically harmful outcomes. Until now, adult crying has received relatively little interest from investigators, whereas in the popular media there are many strong claims about crying of which the scientific basis is not clear. In this review, we provide an overview of the current state of the scientific literature with respect to crying. We identify gaps in knowledge and propose questions for future research. The following topics receive special attention: Ontogenetic development, antecedents, individual and gender differences, and the intra- and interindividual effects of (...) crying. We conclude that the study of crying may help us obtain better insight into human nature, that is, not only our emotional, but also social, and moral functioning. (shrink)
